Collect the URL, symptoms, scope, and recent changes for a focused investigation.

Identify the page and action

Include the exact affected page or application URL and what you were trying to do. “Submit on the contact page shows a 500 error” is more useful than “The website is broken.” Remove private tokens from URLs before sending them.

Check the scope

  1. Record the browser and device.
  2. Note whether you were signed in.
  3. Identify whether one page or the whole site is affected.
  4. Compare another device or connection if practical.
  5. Record the error and time before refreshing.

Use a test submission with clearly synthetic information when a form needs checking. Do not place real customer data in repeated test attempts.

Explain recent changes

Mention recent content, application, plugin, DNS, certificate, or hosting updates if you know about them. If nothing changed to your knowledge, say so. You do not need to make additional configuration changes to produce a useful report.

Describe business impact

Explain whether visitors can browse, customers can complete the main workflow, and employees have a workaround. Choose priority accordingly and follow your agreed escalation route for a serious active interruption. Attach a redacted screenshot or short error excerpt, then keep follow-up information on the same ticket.
